Job summary

Journey Hospice in New Jersey seeks a Per Diem Nurse Practitioner to support hospice patients in Gloucester and Burlington Counties. The NP will perform face-to-face medical examinations and palliative care visits to determine Medicare Hospice Benefit eligibility after six months of care, under supervision of the Hospice Medical Director.

Qualifications

• Graduation from a master's-level program designed to educate and prepare nurse practitioners/clinical nurse specialists.
• Completion of a graduate level pharmacology course (45 hours or 3-credit) integrated into the MSN program or equivalent.
• Currently licensed and nationally certified as an Advance Practice Nurse in New Jersey.

Responsibilities

• Perform patient medical examinations and palliative care visits to certify Medicare Hospice Benefit after six months of hospice care.
• Document visits in the patient’s clinical record and the required face-to-face forms.
• Provide medical care and palliative care interventions for hospice patients on an as-needed basis.
• Communicate effectively with the Medical Director, Administrator and the Hospice Interdisciplinary Team.
