Job summary

Agrace in Wisconsin seeks a skilled Nurse Practitioner, Hospice/Palliative, to deliver community-based palliative care in collaboration with the patient’s healthcare team. The NP evaluates symptoms, plans care goals, and supports families in decision-making.

You will participate in on-call rotations for hospice, coordinate with physicians and other care providers, and contribute to quality improvement within Agrace’s interdisciplinary team.

Qualifications

• Current, unrestricted RN licensure and NP certification in state of employment.
• APN/Nurse Practitioner certification with DEA licensure is required.
• CPR certification within 90 days of hire.

Responsibilities

• Provide clinical care in community-based Palliative Care Program.
• Follow up in person or via telehealth.
• Lead discussions on goals, preferences, and values with patients and families.
• Coordinate with PCPs and care teams to ensure comprehensive care.
• Participate in on-call rotation for hospice team.
