Job summary

Qualifications

Graduate from an accredited school of nursing and accredited Nurse Practitioner program.
Master’s degree with a specialty certification preferred.
Current nursing license in the state of Michigan.
Minimum of one year hospice or palliative care experience.
Excellent organization and time management skills.
Proficient computer knowledge and skills; strong observation, verbal and written communication.

Responsibilities

Provide medical care and manage terminal illness under collaborating physician supervision.
Complete physical exams and interpret lab/diagnostic results.
Develop individualized care plans and coordinate hospice services.
Perform hospice face-to-face visits to assess eligibility.
Collaborate with Chief Medical Officer/physician on eligibility and certification.
Communicate and coordinate with hospice interdisciplinary team on patient plans of care.
Educate patients and families on care, prevention and wellness.
