Residential Home Health and Hospice in Troy, MI is hiring a Nurse Practitioner for a Palliative Care position providing symptom management and compassionate care at patients’ homes. You will work with families and an interdisciplinary team to enhance quality of life and align care with goals.

Expect independent practice with supported growth, and frequent home visits within your territory. We value strong clinical judgment and thorough EMR documentation.

Formación

• BSN and MSN from accredited programs.
• Minimum five years of clinical experience.
• Active RN and NP licenses in the state of work.
• Home health or hospice experience preferred.

Responsabilidades

• Deliver compassionate, patient-centered palliative care, partnering with patients, families, and interdisciplinary teams to improve quality of life.
• Perform comprehensive assessments, manage symptoms, and develop individualized treatment plans.
• Facilitate goals-of-care discussions and educate patients and caregivers on options.
• Work independently, but collaborate with the care team for coordinated care.
• Travel to patient homes within the assigned territory and maintain EMR documentation.
