Job summary

AdvisaCare Hospice & Palliative Care in Michigan seeks a Nurse Practitioner to join our team. You will provide compassionate, patient-centered care to people facing life-limiting illness in home settings, perform assessments, order tests, prescribe medications within scope, and coordinate with the interdisciplinary team.

This role emphasizes collaboration with the Medical Director, education for patients and caregivers, and ongoing professional growth.

Qualifications

• Current unrestricted Nurse Practitioner in Michigan.
• Minimum of one-year experience in Medicare-certified home health/hospice agency preferred.
• Knowledge of home health nursing/hospice services preferred.
• Willing and able to meet the agency need to support Medical Director/designee.
• Understanding of confidentiality and professional ethics.

Responsibilities

• Perform routine and urgent assessments in home care settings.
• Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
• Prescribe medications including controlled substances to the extent delegated and licensed.
• Order treatments and durable medical equipment as indicated.
• Assists patients and families in identification of goals of treatment and ongoing plans of care at every visit.
• Collaborates with palliative care physician or designees as needed and informs primary physician of services.
• Maintains accurate patient records and supports reimbursement for services provided.
• Ensures continuity of care by communicating with the multidisciplinary care team.
• Participates in Quality Assurance activities and adheres to policies/procedures.
• Provides training and onboarding support for staff.
