Heartland Hospice seeks a Nurse Practitioner to provide comprehensive, patient-centered hospice/palliative care, with potential involvement in hospice visits, F2F encounters for eligibility, and collaboration with IDG/IDT to ensure compliant, quality care.

You will work autonomously yet collaboratively, guiding patients and families through complex decisions, symptom management, and end-of-life planning across settings including home and facilities.

Competenze

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Mansioni

• Perform Medicare hospice F2F visits within CMS timeframes.
• Collaborate with Hospice and Palliative Medical Directors and IDT.
• Coordinate with PCP/Attending Provider and specialists to plan care.
• Conduct in-depth patient assessments and manage care plans.
• Prescribe medications and treatments within scope and APAs.
• Order and interpret tests within allowed scope.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across home, SNFs, and assisted living.
• Document findings per regulatory standards.
• Participate in on-call rotation.
