Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner to deliver compassionate care for seriously ill patients in Rutherfordton, NC. You will join a supportive team focused on aligning treatments with patient goals and improving quality of life.

You will provide ongoing palliative care and hospice services, collaborate with clinicians, and engage in education for patients, families, and the care team. Opportunities for professional development are available.

Qualifications

• Graduate of an accredited nursing program with NP certification/licensure.
• Minimum 2 years of relevant chronic disease/long-term care experience preferred, hospice background a plus.

Responsibilities

• Provide hospice and palliative care to seriously ill patients.
• Collaborate with multidisciplinary team to coordinate care across settings.
• Educate patients and families on care goals, treatments, and symptom management.
