Job summary

Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner to care for seriously ill patients in their homes. You will alleviate burdensome symptoms, align treatments with patient goals, and collaborate with a dedicated team of clinicians to deliver compassionate care.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively

Responsibilities

• Care for seriously ill patients to alleviate burdensome symptoms and align treatments with patient goals and values
• Collaborate with a team of clinicians to deliver care in patients' homes
• Provide ongoing palliative care and hospice education to patients and families
• Be an active member of the medical community in your territory with leadership support and development opportunities

Education

Nursing degree (Certificate, Diploma, Associate or Baccalaureate School of Nursing)
