Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to join an interdisciplinary team delivering advanced palliative care to patients in their homes. You will work autonomously under the Palliative Medical Director, guiding patients and families through every stage of treatment to achieve the best possible quality of life.

Qualifications

• Master of Science in Nursing (MSN) from an accredited school.
• Minimum two years of Nurse Practitioner experience.
• Family or Adult Medicine NP specialty preferred.
• Experience with Part B billing.
• Ability to work autonomously in an AP role.

Responsibilities

• Collaborate with Palliative Medical Director and care team to deliver comprehensive, patient-centered care.
• Perform in-depth patient assessments, including physical, emotional, and psychosocial evaluations.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient needs and progress.
• Prescribe medications and order diagnostic tests as permitted by state law.
• Maintain timely, accurate clinical documentation per standards.
• Communicate effectively with physicians, caregivers, and healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
