Job summary

Dartmouth Health at Home seeks an APRN to lead palliative and hospice care in home settings. The APRN provides holistic assessments, manages pain and symptoms, serves as attending provider, and collaborates with the hospice medical team.

The role includes education of patients, families, and facility staff, and participation in the education of fellows and rotating learners within Dartmouth-Hitchcock's Section of Palliative Medicine.

Qualifications

• Board certification as APRN and active national NP certification.
• Eligible for NH and VT licenses and DEA.
• Advanced palliative care fellowship or 2+ years hospice/palliative care experience.
• Certified in Advanced Practice Palliative Nursing or eligible within 1 year.

Responsibilities

• Evaluate, assess and manage Palliative Care needs, including pain and symptom management.
• Provide education to patients and their family, according to the plan of care.
• Work effectively with other health care professionals in home-based settings and facilities.
• Assist with diagnosis, treatment and management of acute and chronic health conditions.
• Consult with Chief Medical Officer and inform primary provider of recommended services.
• Educate facility staff, co-workers and the community on home-based palliative care and hospice services.
