Job summary

Community Healthcare of Texas is seeking a Nurse Practitioner to provide Hospice care to geriatric and pediatric patients in collaboration with the Hospice Team. You will perform procedures within your scope and document assessments and progress notes.

Qualifications

Responsibilities

• Conduct Face-to-Face visits to assess hospice eligibility.
• Document assessments, evaluations, and progress notes.
• Communicate patient needs requiring medical intervention to RN Case Manager.
• Coordinate visits and scheduling with the Patient Care Manager.
• Maintain regulatory and accrediting compliance.

Position Summary

Works in collaboration with the Chief Medical Director and Hospice Team to provide Hospice care services to geriatric and pediatric patients and families within the scope of practice of a nurse practitioner. Perform procedures for which he/she is privileged to do and document assessments, evaluations, consultation reports, and progress notes.

Essential Functions

• Conduct Face-to-Face visits to assess patients' need for ongoing hospice eligibility.
• Complete required documentation to support eligibility and to meet regulatory requirements for communication of finding to Community Hospice of Texas / Providence Hospice Medical Director.
• Communicate to RN Case Manager any patient needs requiring medical intervention.
• Coordinate visits and scheduling with the Patient Care Manager.
• Maintain compliance with all regulatory and accrediting agencies.
• Responsible for demonstrating an appreciation and understanding of the philosophy, mission, and service standards of Community Hospice of Texas and for providing excellent customer service.

Position Qualifications
