Job summary

St-Croix-Hospice- is seeking an On-Call Registered Nurse to organize and direct hospice care, covering nights and weekends. You will perform comprehensive patient assessments, develop and revise plans of care, administer medications, and educate families in home settings.

Must have RN licensure and CPR certification, with 1–2 years of acute and home care experience. A valid driver's license and strong communication skills are essential for coordinating with physicians and the interdisciplinary

Qualifications

Graduate of an accredited school of nursing.
One to two years of acute care experience, plus home care experience.
Current State licensure and CPR certification.
Must be a licensed driver with insured vehicle.
Excellent communication and problem-solving skills.

Responsibilities

Respond to calls from triage during assigned shifts and visit patients with changing conditions.
Perform initial and ongoing comprehensive patient/family assessments for after-hours care.
Write and initiate plan of care; re-evaluate needs with patient and family.
Administer medications and treatments per physician's orders.
Coordinate with physicians and interdisciplinary team; document notes.
Educate patient and family on self-care and home care strategies.
Provide after-hours on-call coverage and crisis support.
Ensure safe home environments and arrange equipment/services.
Attend interdisciplinary group meetings.
Supervise hospice aides; participate in quality improvement activities.
