Job summary

Join Amedisys as an on-call Hospice Registered Nurse, delivering home-based care with a Mon-Thurs 5pm–8am schedule. A $7,500 sign-on bonus and a salary range of $89,000–$93,000 reflect the value of your expertise.

Responsibilities include initial nursing assessment, coordinating care plans, communicating progress to the care team, educating families, and on-call coverage. Must have an RN license for the state(s) you work in, CPR certification, and reliable transportation; 1+ year nursing

Qualifications

• Current RN license for the state(s) you work in.
• 1+ year of nursing experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Educates families on patient condition, needs and care.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
