Job summary

KeystoneCare is a nonprofit home health and hospice provider serving the Greater Philadelphia region. We seek a Hospice On-Call RN to deliver urgent after-hours nursing support, providing calm, clinical judgment and close coordination with the care team to keep patients comfortable and supported.

Responsibilities include on-call assessments, documentation in the EMR, coordinating medications and equipment with community providers, and guiding patients and families by phone.

Qualifications

• Nursing degree and PA RN license.
• 1–2 years hospice or palliative care nursing experience.
• Current unrestricted RN license in Pennsylvania.
• Valid driver’s license and reliable vehicle.
• Eligibility to work in the United States.
• Availability for Monday through Thursday and Friday evening on-call coverage.
• Availability for Friday evening through Monday morning on-call coverage.
• Active CPR certification is a plus.

Responsibilities

• Provide on-call hospice support to patients and families after regular hours.
• Assess urgent symptoms and coordinate timely interventions with the interdisciplinary care team.
• Respond to patient and caregiver calls with compassionate clinical guidance.
• Document on-call assessments, interventions, and outcomes in the electronic medical record.
• Coordinate emergency medication, equipment, and service needs with community providers.
