Job summary

KeystoneCare is seeking a compassionate Hospice On-Call Registered Nurse to provide weekend support for our hospice patients and their families. This critical role ensures patients receive expert care during off-hours, helping manage symptoms and support families through end-of-life care.

You will be part of an interdisciplinary hospice team, responding to urgent patient needs, coordinating care, and ensuring seamless service delivery in the Commonwealth of Pennsylvania.

Qualifications

• Current RN license in good standing to practice in Pennsylvania.
• Hospice or palliative care experience preferred.
• Excellent clinical assessment and symptom management skills.
• Strong communication and interpersonal skills.
• Ability to respond promptly and independently during on-call shifts.
• Driver's License and reliable vehicle.
• CPR certification.

Responsibilities

• Respond promptly to on-call hospice patient needs during weekends.
• Assess patient conditions, manage symptoms, and provide nursing interventions.
• Provide guidance and support to patients' families over the phone or in person as needed.
• Collaborate with the hospice interdisciplinary team to ensure continuity of care.
• Document all patient interactions, assessments, and interventions accurately and timely.
• Adhere to hospice philosophy and policies in all aspects of care delivery.
• Admit patients who are hospice appropriate and interact with families in assessing needs and treatment plan.
