KeystoneCare is seeking a compassionate Hospice On-Call Registered Nurse to provide weekend support for our hospice patients and their families in Pennsylvania. This role delivers expert symptom management, comfort, and guidance during off-hours, coordinating with the interdisciplinary team to ensure seamless care.

You will respond promptly to urgent patient needs, document care interactions, and support families by phone or in person.

Qualifications

• Current RN license in good standing to practice in Pennsylvania.
• Hospice or palliative care experience preferred.
• Excellent clinical assessment and symptom management skills.
• Strong communication and interpersonal skills.
• Ability to respond promptly and independently during on-call shifts.
• CPR certification.

Responsibilities

• Respond promptly to on-call hospice patient needs during weekends.
• Assess patient conditions, manage symptoms, and provide necessary nursing interventions.
• Provide guidance and support to patients' families and caregivers.
• Collaborate with the hospice interdisciplinary team to ensure continuity of care.
