Job summary

KeystoneCare is seeking a compassionate Hospice On-Call Registered Nurse to provide weekend coverage for our hospice patients and their families in the Philadelphia area. You will respond to urgent patient needs, assess symptoms, and deliver nursing interventions during off hours.

You will collaborate with the interdisciplinary team to coordinate care, document all interactions, and support families over the phone or in person.

Qualifications

• Current RN license in good standing in Pennsylvania.
• Hospice or palliative care experience preferred.
• Excellent clinical assessment and symptom management skills.
• Strong communication and interpersonal skills.
• Ability to respond promptly during on-call shifts.

Responsibilities

• Respond promptly to weekend on-call hospice needs.
• Assess conditions, manage symptoms, and provide nursing interventions.

This critical role ensures that patients receive expert care and guidance during off-hours, helping to manage symptoms, provide comfort, and support families through the challenges of end-of-life care.

As an on-call nurse, you will be a vital member of our interdisciplinary hospice team, responding to urgent patient needs, collaborating with physicians and staff, and assisting with care coordination to ensure seamless service delivery.
