Job summary

KeystoneCare is seeking a compassionate Hospice On-Call Registered Nurse to provide weekend support for our hospice patients and their families. This critical role ensures that patients receive expert care, symptom management, comfort, and guidance during off-hours, helping families navigate end-of-life decisions with dignity.

The on-call RN collaborates with the interdisciplinary team, adheres to policy, documents care accurately, and supports continuity of care across shifts in Pennsylvania.

Qualifications

• Active RN license in PA (in good standing)
• Hospice or palliative care experience preferred
• Strong clinical assessment and symptom management skills
• Excellent communication and interpersonal abilities
• Ability to respond promptly during on-call shifts
• CPR certification
• Driver's license and reliable vehicle

Responsibilities

• Respond promptly to weekend on-call hospice patient needs
• Assess patient conditions, manage symptoms, and perform nursing interventions
• Provide guidance and support to patients' families over the phone or in person
• Collaborate with the hospice interdisciplinary team to ensure continuity of care
• Document all patient interactions, assessments, and interventions accurately
• Adhere to hospice philosophy and policies in all aspects of care delivery
• Admit patients who are hospice appropriate and interact with families in assessing needs and treatment plans
