Job summary

Qualifications

• Registered Nurse license in North Carolina
• Minimum of one year of professional nursing experience in acute care
• Evidence of leadership qualities
• Valid driver's license and reliable transportation

Responsibilities

• Coordinate hospice services in assigned counties per the hospice philosophy, goals, and policies.
• Maintain regular electronic documentation and timely EMR entries (within 48 hours).
• Collaborate with supervisor and IDG team to plan and deliver patient-centered hospice care.
• Provide end-of-life care, patient/family education, and bereavement support as needed.
