UnitedHealth Group, through Haywood Hospice, is seeking a Registered Nurse Case Manager to deliver hospice care in patients' homes within Haywood and Jackson counties, NC. This full-time role requires an RN license in NC and involves in-home visits with daytime hours and occasional on-call rotations.

Salary ranges from $73,000 to $82,000 annually, with a $5,000 sign-on bonus for external candidates. Travel and a comprehensive benefits package are provided.

Qualificações

• Current and unrestricted RN licensure in North Carolina.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsabilidades

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care and documents progress.
• Collaborates with patient/family and healthcare team to deliver daily care.
• Provides patient/family instruction for self-care techniques and maintains accurate clinical notes.
