3HC Home Health and Hospice in Wilson, NC is seeking a Hybrid HH & Hospice Nurse, RN (PRN) to deliver skilled nursing care in patients' homes and coordinate hospice services in accordance with agency policies.

Requirements include NC RN license, at least one year in acute care, CPR/BCLS, EMR experience, strong communication, dependable, and valid driver's license; hospice experience preferred. Travel within the service area is expected and compensation is on an as-needed basis.

Qualifications

• Currently licensed as a Registered Nurse in North Carolina.
• Minimum of 1 year of professional nursing experience in an acute care setting.
• Prior hospice or terminally ill patient experience preferred.
• Maintain current CPR certification (BCLS).
• Basic computer skills and experience with electronic medical records.

Responsabilités

• Provide skilled nursing care per the physician's plan of treatment.
• Coordinate hospice services and interdisciplinary care.
• Document patient care and maintain accurate EMR records.
• Educate patients and families on care plans and home safety.
• Travel to patient homes within the service area; maintain reliable transportation.
