Job summary

3HC Home Health and Hospice in Wilson, NC is seeking an experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The Home Health Nurse, RN will work full-time, typically Monday through Friday, serving patients in the Wilson area.

The ideal candidate is an RN licensed in North Carolina with a background in acute care, capable of coordinating care, educating patients and families, and collaborating with a multidisciplinary team to achieve optimal patient

Qualifications

• Required: Registered Nurse licensed to practice in North Carolina.
• Minimum of 1 year experience as a professional nurse in an acute care setting.
• Basic computer skills/experience with electronic medical records (EMR).

Responsibilities

• Provides skilled nursing care as outlined in the physician's plan of treatment and 3HC policies.
• Observes and documents signs and symptoms and reports changes to the physician and other appropriate staff.
• Ensures ongoing evaluation of patient and family needs and discharge planning from services.
• Provides regular supervision to LPNs and paraprofessionals according to CMS/Hospice guidelines; coordinates care with the team.
• Travels to patient homes and collaborates with the supervisor to meet productivity standards.
