Job summary

North Country Healthcare in New Hampshire seeks a Hospice Advanced Practice Registered Nurse (APRN) to deliver comprehensive palliative and hospice care. You will evaluate patients, manage symptoms, develop care plans, and educate patients and families, coordinating with the Hospice Medical Director, physicians, and the interdisciplinary team.

The APRN promotes quality end-of-life care aligning with patient goals, while ensuring CMS Hospice Conditions of Participation compliance and applicable

Qualifications

  • Master’s or Doctoral Degree in Nursing from an accredited program.
  • National Nurse Practitioner Certification.
  • Hospice and Palliative Care Certification preferred.
  • BLS/ACLS/PALS training (renewal required).
  • Licensed as an APRN in New Hampshire; DEA license.
  • Valid driver’s license and reliable transportation.
  • EMR experience preferred.
  • Two years hospice/palliative care experience preferred.
  • Home health/community-based care experience preferred.

Responsibilities

  • Provide compassionate, patient-centered hospice and palliative care to patients with terminal illnesses.
  • Perform comprehensive hospice and palliative care assessments.
  • Educate patients and families on disease progression, symptom management, and goals of care.
  • Develop and revise individualized care plans; order/interpret tests as appropriate.
  • Complete hospice recertification documentation and required face-to-face assessments.
  • Coordinate transitions of care and participate in interdisciplinary team meetings.
  • Document all encounters per agency standards and ensure regulatory compliance.
  • Support a culture of safety, quality improvement, and patient/family satisfaction.
