Job summary

Elara Caring in Kokomo, Indiana seeks a Hospice Nurse Practitioner to lead clinical care for hospice patients in the home setting. You will conduct face-to-face visits, assess symptoms, document findings, and collaborate with physicians to implement the Plan of Care with a focus on quality, compassion, and timely in-home support.

You will provide training to staff, participate in guideline development, and ensure CMS-compliant documentation while maintaining the highest standards of palliative

Qualifications

• Current NP license in the practicing state (Indiana)
• Master's degree in nursing and NP certification
• 1+ years clinical nursing experience
• DEA certification if allowed to prescribe controlled substances
• Valid driver's license or reliable transportation for travel to patient homes

Responsibilities

• Perform routine and urgent face-to-face hospice visits including admissions and transfers
• Assess patient needs, document findings, and coordinate with physicians and care team
• Provide bedside palliative care and support to patients and families
• Triage needs to prevent unnecessary hospitalizations and manage home-based care
• Maintain accurate patient records, billing, and CMS-compliant documentation
• Collaborate with Medical Director and interdisciplinary team to ensure quality care
• Provide training to clinical staff and help develop clinical guidelines
