Job summary

Elite Hospice, part of LHC Group and a nationwide post-acute care partner within the Optum family, is seeking a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management and supportive care for hospice patients as requested by the referring physician.

Primary responsibilities include face-to-face visits within prescribed time limits, coordinating plans of care with the Hospice Medical Director and other physicians, educating patients and families, documenting care

Qualifications

• Active, unrestricted nursing license in the state of practice.
• Current NP certification and CNS credentials in the state of practice.
• National board certifications (ANCC/NBCHPN/AANP) are preferred.

Responsibilities

• Conduct face-to-face visits with hospice patients within required timeframes.
• Consult with Hospice Medical Director and coordinate with treating physicians.
• Initiate and coordinate the plan of care and document patient/family responses.
• Instruct and support patient/family in self-care when appropriate.
• Maintain accurate clinical notes and report needs to Administrator and Medical Director.
