Qualifikationen

• Two years experience as an Advanced Practice Nurse, hospice or home health.
• Current RN license and Advanced Practice Nurse license.
• Driver’s license.

• Collaborates with the IDG/Team to ensure palliative care planning and evaluation.
• Consults with and educates patients/families about disease process and care options.
• Performs and documents home visits and face-to-face encounters as required.
• Provides medical direction and education to staff and communicates with physicians.
