Job summary

Elara Caring is seeking a Hospice Nurse Practitioner to join our team delivering compassionate, home-based hospice care. You will perform recertifications, assess patient needs, and collaborate with physicians to manage symptoms and the Plan of Care.

The role requires NP licensure in the state, a Master’s in nursing, NP certification, and 1+ year of clinical nursing experience. DEA certification is required where permitted.

Qualifications

• Current licensure as a Nurse Practitioner in the state of practice.
• Master’s degree in nursing and NP certification.
• 1+ years of clinical nursing experience.
• Maintain active DEA certification if state allows prescribing controlled substances.
• Ability to lift up to 50 pounds and move equipment.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients including admissions and transfers.
• Assess patient needs, document findings, and collaborate with physicians to manage symptoms.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs to prevent avoidable hospitalizations by home visits.
• Maintain accurate patient records, billing, and coding per CMS guidelines.
• Collaborate with Medical Director and team to coordinate quality patient care.
• Provide training and continuing education to clinical staff.
• Develop clinical practice guidelines for high-quality hospice care.
