Job summary

Eternal Faith Hospice Care is seeking a Nurse Practitioner to assist physicians in overseeing clinical care for patients and families, including face-to-face documentation and home visits for palliation and management of terminal illness.

You will collaborate with the Interdisciplinary Group/Team, provide education to patients and staff, perform visits, and contribute to medical direction and high-quality, compassionate hospice care.

Qualifications

• NP degree (MSN/ Doctoral) required or equivalent.
• Active RN license in state and APRN licensure.
• hospice/home health experience preferred.

Responsibilities

• Collaborates with the Interdisciplinary Group/Team to ensure appropriate evaluation and palliation.
• Consults with patients/families about care plans and education.
• Performs and documents home visits for palliation and end-of-life care.
• Provides medical direction and education to clinical staff.
• Communicates changes in patient needs to physicians and team.
