Job summary

Four Pillars Hospice seeks a Nurse Practitioner to oversee clinical care for patients and families, consulting with the hospice physician and guiding the Interdisciplinary Group to ensure quality care.

You will collaborate with the IDG/T, develop care plans, educate patients and families about disease and end-of-life care, perform visits, and support on-call needs while adhering to HIPAA and hospice policies.

Qualifications

• Nurse Practitioner license and RN license required.
• Strong knowledge of pharmacology, palliative care and end-of-life.
• Ability to travel locally and work flexible hours.

Responsibilities

• Collaborate with IDG/T for evaluation and palliation.
• Develop and educate patient/family care plans to ensure quality care.
• Educate patients and families about disease, self-care, and end-of-life care; train staff as needed.
• Perform and document patient/family visits per care plan.
• Coordinate with physicians on palliation and consults.
