Job summary

St. Gabriel's Hospice and Palliative Care in Fort Worth, TX seeks a Nurse Practitioner to support physicians in delivering compassionate end-of-life care. The NP conducts face-to-face visits with patients and families, coordinates palliation plans, and provides clinical direction to the Interdisciplinary Group.

You will educate families, document assessments, and collaborate with the hospice physician to adapt care plans. A valid NP license and RN license, plus hospice experience, are preferred.

Qualifications

• Collaborates with the IDG/IDT to ensure appropriate evaluation and palliative treatment.
• Educates patient/family on disease process and end-of-life care.
• Documents visits and plans of care.
• Provides clinical direction to staff and physicians as needed.

Responsibilities

• Collaborates with IDG/IDT to ensure appropriate evaluation and palliative treatment.
• Consults with and assists in development of patient/family plan of care to ensure quality care.
• Educates patients and families about disease process and end-of-life care.
• Performs and documents visits to patient/family.
• Communicates with physicians to ensure optimal patient care.
• Provides education to other IDG/T members as needed.
• Completes face-to-face encounters with patients prior to 3rd benefit period.
• Provides medical direction and education to clinical staff.
