Job summary

Four Pillars Hospice in Stephenville, TX seeks a Nurse Practitioner to oversee clinical care and guide the interdisciplinary team in delivering compassionate end-of-life support. You will consult with physicians, participate in care planning, and ensure quality palliation for patients and families.

The role requires NP licensure, local travel, flexible scheduling, and collaboration with the IDG/T. You’ll train staff, conduct face-to-face visits, and help families navigate medical decisions with

Qualifications

RN and APN licensure required for practice.
NP/CRNP degree or doctoral nursing program completed.
Two years' APRN experience in clinical care; hospice/home health preferred.
Strong knowledge of pharmacology, end-of-life care and patient education.

Responsibilities

Collaborate with IDG/T to assure palliation and appropriate care.
Consult with attending physicians and hospice physician for care plans.
Provide education to patient/family on disease, self-care, and end-of-life issues.
Perform and document patient/family visits per plan of care.
Coordinate with other IDG/T members and provide clinical direction as needed.
Communicate changes in patient needs to physicians and staff.
Complete face-to-face encounters prior to 3rd benefit period and subsequently.
