Job summary

Gentiva seeks a Nurse Practitioner to provide comprehensive, patient-centered hospice and palliative care. You will perform F2F visits for eligibility and recertification, collaborate with physicians and IDGs, and guide patients and families through complex medical decisions and end-of-life care.

You will manage symptoms, coordinate care across settings, and ensure compliance with Medicare/Medicaid requirements. Must hold NP license and CMS competencies.

Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Perform Medicare-required hospice Face-to-Face visits within CMS timeframes.
• Collaborate with Hospice and Palliative Medical Directors and IDG/IDT.
• Coordinate patient Plan of Care with PCP/Attending Providers and specialists.
• Conduct in-depth patient assessments across physical, emotional, functional, and psychosocial domains.
• Support care plans with patients, families, and caregivers.
• Prescribe medications and treatments as permitted by state law and APP agreement.
• Order and interpret laboratory/tests per state scope of practice.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across home, SNF, and assisted living settings.
