Job summary

Amedisys, Inc. is seeking a qualified RN Case Manager to join our home health care team in Tennessee. You will assess patients, collaborate with the care team, and develop individualized care plans to support patients at home.

Responsibilities include coordinating referrals, maintaining progress records, and teaching patients and families care techniques. On-call duties and full nursing duties are part of the role.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate care techniques.
• Communicates regularly with clinical manager and care team.
• Performs on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
