Qualified candidates will bring NP licensure, BSN/MSN degrees, and at least five years of clinical experience, preferably in home health or hospice settings. Travel is required within the territory.

Qualifications

• Current RN and NP licenses in the applicable state.
• Minimum five years of clinical experience.
• Experience in home health or hospice/palliative care preferred.

Responsibilities

• Provide compassionate hospice NP care in patients' homes in the New Kensington area.
• Conduct comprehensive assessments and manage complex symptoms and plans of care.
• Educate patients and families while ensuring compliant EMR documentation.
• Work independently within an interdisciplinary team to support end-of-life goals.
• Travel to patient homes within assigned territory.
