Job summary

Harmony Hospice is seeking a Nurse Practitioner for a full-time position in Rhode Island. The NP will provide advanced clinical care, symptom management, and leadership in end-of-life care across settings, including telehealth when suitable.

Requirements include an accredited NP program, active RI NP license, DEA license, BLS/CPR, and a minimum of one year in nursing practice with hospice or palliative care experience. A master's degree is required and travel to patient sites is expected.

Qualifications

Graduate of an accredited School of Nursing and accredited Nurse Practitioner program.
Current Rhode Island NP license.
Current DEA license.
Current BLS/CPR certification.
Master's degree required.
Minimum of one year of Nurse Practitioner experience.

Responsibilities

Provide advanced medical care and clinical support to hospice patients and their families.
Evaluate, diagnose, and manage pain, symptoms, and terminal illnesses in line with hospice best practices.
Perform comprehensive patient assessments and determine eligibility and prognosis for hospice services.
Conduct patient visits in private residences, SNFs, assisted living, hospitals, and telehealth as appropriate.
Collaborate with attending physicians and the interdisciplinary team to coordinate care plans.
Communicate with referral sources and healthcare providers regarding new patient referrals.
Complete required face-to-face encounter documentation and maintain accurate clinical records.
Educate patients, families, caregivers, and staff on disease progression and hospice services.
