Job summary

AccentCare, Inc. in Rhode Island is seeking a Hospice Nurse Practitioner for a PRN/Part-Time role. You will coordinate patient care with physicians and the hospice team to ensure compassionate, high-quality services in patients' homes or facilities.

The position requires a Masters degree, an active APRN license, at least 1 year of NP experience, certification by a recognized NP board, and CPR (preferred). A valid driver's license is also needed to travel between locations.

Qualifications

• Graduated from an approved school of professional nursing - Masters degree.
• Current and unencumbered advanced practice nurse license in the state(s) practicing.
• One year of experience as an NP
• Certified by a nationally recognized advanced nurse practice organization
• Possess and maintain valid CPR certification while employed in a clinical role (preferred)
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which the program provides services

Responsibilities

• Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
• Monitor patient progress; update the Team Director and team members; consult regarding discharge planning and patient/family teaching
• Maintain continuity of care in coordinating with the care team and case manager for patients with ongoing needs
• Prescribe medication as permitted by the state and/or the state Board of Nursing
