Job summary

Crossbridge Hospice, LLC is seeking a Nurse Practitioner to conduct face-to-face hospice assessments, collaborate with the care team, and ensure patient needs are met with compassionate, high-quality care. The role emphasizes aging‑friendly practices and close coordination with RN Case Managers and physicians.

The ideal candidate holds a Master’s in Nursing, an active NP license in the state, and hospice/geriatrics experience is preferred.

Qualifications

• Master’s Degree in Nursing is required.
• Current unrestricted Nurse Practitioner license in state of practice.
• Board Certification preferred.

Responsibilities

• Conduct Medicare-required face-to-face assessments and clinical visits for hospice patients.
• Collaborate with RN Case Managers, interdisciplinary team, and sponsoring physician to meet patient needs.
• Document clinical findings and communicate with physician responsible for recertification.
• Submit time and mileage logs for patient-related activities every two weeks.
• Apply the 4Ms framework (What Matters, Medications, Mentation, Mobility) to provide Age-Friendly Care.
