UnitedHealth Group is seeking a Hospice Nurse Practitioner in Florida to provide compassionate, patient-centered end-of-life care within an interdisciplinary hospice team. You will focus on comfort, dignity, and quality of life, collaborating with physicians, nurses, and social workers to support patients and families.

The role requires an active NP license in the state, Medicare billing privileges, and CPR certification, with responsibilities including assessment, symptom management, and care

Competenze

• Current Nurse Practitioner license in the state of assignment.
• Minimum 1 year as an NP or 1 year as hospice RN in last 3 years.
• Authority to practice as an NP and to prescribe in the state.
• Must have Medicare billing privileges as an NP.
• Must meet state physician–NP collaborative requirements and provide active agreements.
• Must have state-required insurance to practice or be eligible through employer coverage.
• Knowledge of Medicare reimbursement and coding for various settings.
• Current CPR certification.

• Communicate significant findings and changes to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters, assess changes in condition, function, and quality of life.
• Order and review labs, imaging, EKGs; document treatment plan and review results with physician.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
