Job summary

UnitedHealth Group is seeking experienced hospice nurses to perform face-to-face visits and coordinate patient care in collaboration with physicians and the Hospice Medical Director.

Licensed Advanced Practice Nurses (APNs) or Nurse Practitioners with 5+ years of nursing experience, CPR certification, and an unrestricted state license are preferred. Compensation reflects local markets and full-time benefits are provided.

Qualifications

• Active unrestricted license in the state of residence
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
• Current certification by ANCC, NBCHPN, or AANP
• 5+ years nursing experience
• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings
• Consults with the Hospice Medical Director and informs primary physician of recommended services
• Initiates and coordinates the plan of care, documents problems, goals, interventions and responses
• Instructs and supervises the patient/family in self-care techniques when appropriate
• Maintains accurate clinical notes and reports patient condition to Administrator and Hospice Medical Director
