Four Pillars Hospice is seeking a Nurse Practitioner to oversee clinical care and coordinate palliation with the IDG/T. You will consult with the hospice physician, direct the IDG/T, and ensure high-quality end-of-life care for patients and families.

The role emphasizes education, rapid assessment, and collaboration across the care team to deliver compassionate, patient-centered palliation in a home- or facility-based setting.

Qualifications

  • Graduate from a BSN program and an NP-focused master's or doctoral nursing program.
  • Current state RN license and Advanced Practice Nurse license.
  • Two years of experience as an Advanced Practice Nurse; hospice/home health preferred.
  • Strong pharmacology knowledge and palliative care expertise.
  • Reliable transportation and valid auto liability insurance.
  • Ability to work a flexible schedule and travel locally.
  • Ability to remain calm under pressure and make quick decisions.

Responsibilities

  • Collaborate with the IDG/T to evaluate and provide palliation for terminal disease.
  • Consult with IDG/T to develop patient/family care plans ensuring quality care.
  • Educate patients and families on disease processes, self-care, and end-of-life care; train staff as needed.
  • Perform and document patient/family visits per plan of care and needs.
  • Interact with attending and consulting physicians regarding palliation with hospice physician.
  • Educate IDG/T members on palliation basics as needed.
  • Participate actively in the IDG/T; communicate effectively at all times.
  • Inform physicians of changes in patient needs and outcomes of interventions.

Position Summary

The Nurse Practitioner's Primary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients' attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care

Key Responsibilities
  • Collaborate with the IDG/T in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consult with and assist in the IDG/T's development of patient/family plan of care to assure quality care is provided.
  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
  • Perform and document visits to patient/family based on plan of care and patient/family need.
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
  • Consult with and provide education to other IDG/T members on a prn basis for clinical issues relating to palliation.
  • Activelyparticipate in the IDG/T. Communicate effectively, timely, and appropriately at all times.
  • Inform physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
  • Complete face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
  • Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
  • Always uses standard precautions, and per hospice policy.
  • Adheres to hospice policies regarding protected health information.
  • Keep up with and maintains education/knowledge in pharmacology and palliative medicine.
  • Provide on-call services to patients/families as assigned.
Qualifications
  • Graduate from a Baccalaureate School of Nursing, a Nurse Practitioner (NP) focused Master's or Doctoral Nursing Program
  • Currentstate license as a Registered Nurse, and as an Advanced Practice Nurse. Current driver's license.
  • Twoyears' experience as an Advance Practice Nurse, clinical care setting preferred; hospice or home health preferred.
  • Nursingskills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills..
  • Transportation: Reliable transportation and valid auto liability insurance.
