Franklin VNA in New Hampshire is seeking a full-time Nurse Practitioner to join our hospice team in an in-person capacity. The NP will provide advanced clinical care, symptom management, and palliative support in coordination with the Medical Director and inter-disciplinary team.

Responsibilities include eligibility assessments, documentation, and participation in IDG meetings to uphold hospice standards and compassionate end-of-life care for patients and families in the Franklin area.

Formación

• Current, unrestricted Nurse Practitioner license (state-specific).
• Board certification in a relevant NP specialty (FNP/ANP/AGNP).
• Active DEA license and prescriptive authority.

Responsabilidades

• Perform comprehensive assessments of hospice patients in home and residential settings.
• Assist with hospice admissions by evaluating clinical eligibility and initial med reviews.
• Act as the Attending Physician when chosen.
• Manage pain and complex symptoms (e.g., dyspnea, agitation, delirium, nausea).
• Develop and revise individualized plans of care in collaboration with the hospice physician.
• Order and prescribe medications, DME, and treatments per state scope of practice and hospice policies.
• Respond to acute changes in condition and provide urgent or same-day visits as needed.
• Cover on call for symptom management and med approvals.
• Conduct F2F encounters for eligibility assessments in CMS-required periods.
