Job summary
Heartlite Hospice in Alabama is seeking a Hospice Nurse Practitioner to provide consultation in palliative and supportive care, developing and coordinating patient plans with the care team. You will perform timely face-to-face visits and communicate findings to the Hospice Medical Director and referring physicians.
The role requires an active RN license, CNS/NP certification, and 5+ years of nursing experience.
Qualifications
- Active unrestricted license in the state of residence.
- Current certification as a clinical Nurse Specialist or Nurse Practitioner.
- Current certification by a national body such as ANCC, NBCHPN, or AANP.
- 5+ years nursing experience.
- Current driver's license and vehicle access.
Responsibilities
- Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients.
- Consults with the Hospice Medical Director and collaborates with other physicians as needed.
- Initiates and coordinates the plan of care with patient/family involvement.
- Instructs and supervises patient/family in self-care techniques when appropriate.
- Maintains accurate clinical notes and reports patient status to leadership.
