Job summary

Alabama Hospice of the Shoals, part of the LHC Group and Optum family, seeks a Hospice Nurse Practitioner to provide palliative and supportive care. You will consult with the Hospice Medical Director and coordinate care plans for patients at home and in care settings.

The role emphasizes face-to-face visits, accurate documentation, and collaboration with an interdisciplinary team. Requirements: current NP licensure, certification, 5+ years of nursing, and reliable transportation.

Qualifications

• Active nursing license in the state of practice.
• Current certification as a Nurse Practitioner (APRN/NP).
• 5+ years of nursing experience.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Face-to-face encounter visits within prescribed time limits for hospice patients.
• Consults with Hospice Medical Director and informs primary physician of services.
• Initiates and coordinates the plan of care with patient/family and IDG.
