Heart of Hospice, a part of LHC Group within the Optum family, is seeking a Hospice Nurse Practitioner to provide palliative and supportive care in Baton Rouge, Louisiana. You will conduct face-to-face visits, coordinate plans of care, and collaborate with the Hospice Medical Director and interdisciplinary teams to support patients and families.

Qualifications

• Active unrestricted RN license in the state of residence.
• Current CNS or NP certification in the state of practice.
• Certification by national body such as ANCC, NBCHPN, or AANP.
• 5+ years nursing experience.
• Current driver's license and reliable vehicle or access to transportation.

Responsibilities

• Make face-to-face encounter visits within prescribed time limits for hospice patients.
• Consult with Hospice Medical Director and collaborate with physicians as needed.
• Coordinate plan of care and document patient/family response and progress.
• Instruct and supervise patients/families in self-care techniques.
• Maintain accurate clinical notes and report needs to Administrator and Hospice Medical Director.
