Job summary

St. Croix Hospice is seeking a Nurse Practitioner to provide compassionate, expert medical care for terminally ill patients and their families in Missouri.

The NP will assess, treat, and recertify patients, collaborate with an interdisciplinary team, and guide care decisions aligned with patient goals. The role requires an active NP license in the state, a degree from an accredited NP program, and at least two years of RN experience (hospice experience preferred).

Qualifications

• Must have current NP license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
• Valid driver's license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
