Job summary

St. Croix Hospice in Eau Claire, WI is seeking a compassionate Nurse Practitioner to deliver comprehensive medical care to terminally ill patients and support their families with dignity and comfort.

You will collaborate with interdisciplinary teams, manage medications and recertifications, and develop individualized care plans that honor patient goals while ensuring HIPAA-compliant documentation and continuity of care.

Qualifications

• Must hold a current NP or APRN license in the states of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; hospice or palliative care preferred.
• Provide comprehensive medical care for terminally ill patients, including assessments and care plans.
• Prescribe medications and recertifications; ensure compliance with regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms.
• Educate families on end-of-life care and maintain HIPAA-compliant documentation.
• Valid driver’s license and reliable transportation required.
