Job summary

St. Croix Hospice is seeking a compassionate Nurse Practitioner to support patients and families during the final stages of life. You will manage medications, treatments, and recertifications, while guiding families to align care with patient goals and wishes.

You will collaborate with a multidisciplinary team and ensure documentation complies with HIPAA, maintaining continuity of care and a steady, comforting presence for patients and caregivers.

Qualifications

• Hold a current Nurse Practitioner (NP) license in the state of practice.
• RN experience of at least 2 years; hospice or palliative care preferred.
• Provide comprehensive medical care for terminally ill patients with assessments, interpretations, and care plans.
• Prescribe medications and treatments; complete hospice recertifications and ensure regulatory compliance.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and continuity of care.
• Educate families and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain HIPAA-compliant documentation and engage in quality improvement.

Qualifications

Nurse Practitioner Requirements & Key Responsibilities

• Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
