Job summary

Serenity Hospice PA seeks an experienced Registered Nurse Case Manager to join the Chester County team. You will manage a patient caseload in homes and facilities, delivering expert clinical care with symptom management, education, and emotional support while coordinating the interdisciplinary team for high-quality end-of-life care.

Working 8:30am–4:30pm, Monday to Friday, you will contribute to a compassionate, patient-centered culture in PA.

Qualifications

• Must hold an active Pennsylvania RN license in good standing.
• 1–2 years nursing experience; hospice preferred but not required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Perform initial and ongoing assessments of patients and families to determine hospice needs.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing care, pain and symptom management, and medication administration.
• Educate patients and caregivers on disease process, self-care techniques, and end-of-life planning.
• Coordinate care with physicians, social workers, chaplains, aides, and volunteers.
• Document accurately and timely in electronic medical records.
• Participate in interdisciplinary team meetings.

About the Role:

Serenity Hospice is growing and seeking an experienced and compassionate Registered Nurse Case Manager to join our dedicated team serving patients and families in Chester County, PA.

In this rewarding role, you will manage a caseload of hospice patients in their homes, assisted living facilities, and skilled nursing facilities, providing expert clinical care, symptom management, education, and emotional support while coordinating the interdisciplinary team to ensure the highest quality of end-of-life care.
