Job summary

Serenity Hospice is seeking a dedicated Registered Nurse Case Manager to oversee hospice patients in their homes, assisted living facilities, and SNFs in Chester County, PA.

You will assess needs, lead interdisciplinary care planning, and coordinate with physicians and teams to deliver compassionate end-of-life care. No nights, no weekends, no on-call required; strong clinical and communication skills are essential.

Qualifications

• Current Pennsylvania RN license in good standing.
• 1–2 years nursing experience; hospice/home health or med/surg preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-focused approach with dignity and comfort.

Responsibilities

• Perform initial and ongoing assessments of patients and families to determine hospice needs.
• Develop, implement, and update individualized plans of care.
• Provide skilled nursing care, pain and symptom management, and medication administration.
• Educate patients and caregivers on disease process, self-care techniques, and end-of-life planning.
• Coordinate care with physicians, social workers, chaplains, aides, and volunteers.
• Document accurately in electronic medical records; participate in interdisciplinary team meetings.
