Job summary

Serenity Hospice in Chester County, PA seeks an experienced Registered Nurse Case Manager to join our team. You will manage hospice patients in homes and facilities, delivering expert nursing care, symptom management, and education.

Coordinate with an interdisciplinary team to ensure high-quality end-of-life care in a daytime, weekday schedule. Responsibilities include assessments, care planning, education, and documentation in EMR.

Position: Hospice RN Case Manager

Location: Chester County, Pennsylvania (Southeastern PA Region)

Schedule: Full-Time – Monday through Friday – 8:30am-4:30pm

No Nights No Weekends No On-Call Required

About the Role:

Serenity Hospice is growing and seeking an experienced and compassionate Registered Nurse Case Manager to join our dedicated team serving patients and families in Chester County, PA.

In this rewarding role, you will manage a caseload of hospice patients in their homes, assisted living facilities, and skilled nursing facilities, providing expert clinical care, symptom management, education, and emotional support while coordinating the interdisciplinary team to ensure the highest quality of end-of-life care.

Key Responsibilities

• Perform initial and ongoing assessments of patients and families to determine hospice needs
• Develop, implement, and update individualized plans of care
• Provide skilled nursing care, pain and symptom management, and medication administration

Qualifications

• Current Pennsylvania RN license in good standing
• Minimum of 1–2 years of nursing experience; hospice preferred but not required
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Compassionate, patient-focused approach with a commitment to dignity and comfort
• Reliable transportation, valid driver’s license, and auto insurance
• Ability to work independently while collaborating with a supportive team
